

SURGERY OF HAIR AND FACIAL HAIR TRANSPLANTATION INFORMED CONSENT FORM
The objective of this form is to enable your participation into the decision making process about your healthcare by informing you.
This form has been designed in order to meet the needs of many patients under most conditions, nevertheless, it should not be considered as a document containing the risk of the entire applicable treatment. Based upon your individual healthcare, your physician may provide you with different or additional information.
After acquiring the advantages and possible risks of diagnosis, medical treatment and surgical interventions, it is up to your decision whether or not to accept these procedures. You are entitled to refuse to be informed except for the legal and medical necessities or withdraw your consent at any time.
Information about the Operation
Aesthetic problems may emerge due to hair loss in scalp and face and, psychological disorders can be occured accordingly. This intervention is an aesthetic intervention that covers the treatment of aesthetic problems related to hair loss. If no intervention performed, the existing aesthetic problems will continue.
Before the Operation
• Prior to the operation, you are expected to inform your physician in detail about current and past diseases, drugs used, past operations, alcohol and smoking, known allergies.
• Genetic hair loss characteristic may be progressive and originally haired areas excluding planted parts may continue to lose hair after the hair transplantation due to genetic hair loss disorder and time/severity of loss may not be foreseen by the physician.
• Detailed information will also be given verbally by your physician about the method (FUE) you prefer for hair transplantation.
• In order to have information about the success of the operation, you can talk to the patients previously operated for hair transplantation.
Process Description
• There are two methods of hair transplantation surgery. FUT and FUE (Follicular Unit Extraction meaning the method of harvesting hair follicles from the donor area one at a time). FUT technique includes replacement and proper preparation of the piece harvested from the haired skin of your nape and transplanting it to the needed area.
• FUE technique is the process of harvesting hair follicles one at a time by means of special instruments and transplanting them to the required area without the need for replacing part from your nape. The purpose of this surgery is to gain a better appearance compared to the current state, by means of planting as much hair as possible to the areas where there is a lack or shortage. Most of the time it is impossible to reach a lush hair appearance that seems like no hair is lost. The result depends on the success of surgical method as well as thickness, shape and colour of your hair strands and amount of hair loss. To achieve the best result, two or three sessions of surgery may be required.
• Eyebrow transplantation is the process of transferring the follicles of hair obtained from the nape one at a time, to the area that is short or lack of eyebrows. This method is the same in moustache and beard transplantation.
• Aesthetic surgery operations are health-oriented except that they are generally focused on patient’s satisfaction. For this reason, before the hair transplantation, in the planning phase, tell your desired areas for transplantation to your physician clearly and take your doctor’s opinion into consideration.
• This surgery is to be performed under local anesthesia. Allergies or drug reactions due to anesthesia are rarely seen but can be deadly.
• The total surgery time may vary according to the transplantation area size, the number of collected grafts, and the weakness of the donor site, which may take 4–8 hours.
• Healing process of the wounds lasts between 4–10 days. Swelling of forehead may occur from 3rd day to 5th day. The numbness on the extraction area may last from 3 weeks to a couple of months. Pimples may occur on skin during the hair growth period (between 14th and 22th months).
After the Operation
• After the hair transplantation, 1–2 mm long hair follicles are completely lost in the first two months following the operation date. The planted hair begins to emerge in 4 months after the surgery, growth is completed up to 18 months and the hair reaches its original thickness in 24 months.
• A 10-day personal care period is required. During this time, swelling on the face or eyelids may occur.
• First wash can be done after 48 hours, daily for 15 days.
• Avoid alcohol, smoking, and heavy activities for 15 days.
• Sleep with the head elevated for 4 days.
• Grafts may crust during the first 2 weeks – this is normal.
• Growth rates: 30% by month 4, 70% by month 6, and 90% within 12 months.
• Infection risk exists but is minimized by proper post-op care.
Complications During Operation
• Excessive bleeding may occur rarely.
• Allergic reaction to anesthesia (rare, potentially life-threatening).
• Number of grafts may vary depending on donor area condition.
Complications After Operation
• Scar formation after incision.
• Peripheral hair loss or texture change.
• Temporary sensory loss (topognosis).
• Necrosis in transplanted area (rare).
• Swelling and bruising, redness, and minor infections can occur.
